They will then compare the information provided in the uploaded document(s) against the information entered in the verification form. To ensure a speedy and successful verification, please pay attention to how you enter your information on the form so that it will match what is seen on the document(s). For example, enter your first and last name on the form as it appears on your official document(s).

WHAT KIND OF DOCUMENT(S) DO I UPLOAD IF ASKED?

You’ll need to upload professional documentation that shows your:

This rarely happens. All health administrations are going to give you some kind of official document that we'll accept but, in the event you cannot provide us any acceptable proof of validation, we will be unable to approve you for the offer.
